Why is my dog scratching her lips and nose for a year, causing cuts?

Hi I’m writing today because my dog has been itching & scratching at her mouth for about a year now & she’s constantly giving herself cuts on her lips & her nose, I was wondering if you could tell what it is & also what I can do to help her with it

Its hard to tell what it is from the picture. I have commonly seen yeast and bacterial infections in that area which commonly cause itching. I would try some benadryl twice a day and a medicated wipes like miconahex wipes twice a day to see if it helps. If it isnt improving, especially since it has been going on for a year, I would have your vet take a look. She may need some oral meds and stronger allergy meds to help her heal.
