My unneutered puppy's penis looked alarming. Is this normal?

Updated On September 23rd, 2025

Pet's info: Dog | Mixed Breed | Male | unneutered | 3 months and 12 days old | 23 lbs

Hi, Giacomo is 3 Months old and has not been neutered yet. We are still fostering him and the rescue has not set up an appointment yet. Recently he discovered his penis and it was completely wrecked and it looked like his balls were outside of his body as well. We googled online and put a cold compress on it and it went back in. It’s seems to be normal now, but I just wanted to ask if there is anything else we should follow up with. Could something still be going wrong?

It is likely that he just had an erection. The bilbus glands in the dog are on the sides of the penis and they can become very large during an erection as well. Usually after 15 minutes or so everything will be back to normal. This can still happen even after he is neutered. Best of luck and thank you for using Pet Coach.
