If your dog is self traumatizing herself to the point of bleeding she definitely needs veterinary attention. She likely needs a short course of steroids or an allergy drug called Apoquel to help stop the itch-cycle and get the inflammation under control. Even if you aren't seeing fleas on her, the irritation at the base of the tail is the most common place for flea bites and we can have dogs with flea allergy dermatitis, where just a bite or two by a flea can make them intensely itchy and cause them to self traumatize and in some cases develop secondary bacterial infections that will also require treatment.
