In cats it'a usually due to an intestinal problem, worm infestation can also cause that. the first step would be to deworm him with a high quality dewromer such as Drontal or Profender, if it does not solve the problem you will need to take him to the vet for a checkup and probably other tests such as ultrasound and blood test.

His third eyelids are showing. This can have several causes ranging from infection (bacterial or parasitic), injury, stomach upset or even neurologic issues. Have Basbous physically examined by your veterinarian. Submit a stool sample to diagnose intestinal parasites. Any treatment will depend on the diagnosis.

This is usually caused by a gastrointestinal problem, most commonly intestinal worms. start by deworming him with a high quality dewormer (E.G Drontal), if it doesn't seem to be helping take him to the vet in order to star investigating the problem.

Many times this is the only sign of intestinal worms so I would strongly advise getting him dewormed first thing. If it's not worms it's probably some other type of gastrointestinal discomfort, if it does not go away you will need to get it checked by your vet.
