Your vet can examine a swab of the ears under a microscope in the clinic. Acarexx is a one time treatment. Tresaderm takes about 7 days to clear mite infections. Another option would be Revolution topical. In addition to killing ear mites, it will protect your cat against fleas, heartworm, round worm and hookworm.
