Why does my dog still have fleas on Trifexis and is so itchy?

My dog has been on Trifexsis for 5 months now and I am still having flea problems. I washed him the other night and got 40 fleas off. Also, he's biting, scratching and licking himself constantly. What should i do? Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Male | neutered | 10 months and 5 days old | 35 lbs
