Cat irritable, straining to poop, and has a worm. What should I do?

My cat has been irritable and has been trying to poop and cant and i noticed a worm possibly coming out of his anus

I am sorry to hear about your cat. You should check to be sure your cat has been urinating today. It is common for owners to think their cats are trying to poop when they actually are trying to urinate (going to litterbox and straining with nothing coming out). These cats can initially be crabby/painful/can vocalize, and often react painfully if you press on their belly. As the condition progresses they can become lethargic/vomit/stop eating. The inability to urinate is seen in young male cats and is a medical emergency, so if you aren't sure your kitty is urinating I would recommend a trip to the emergency vet. If you are positive he is peeing, then a trip to your regular vet tomorrow would be recommended. He may need an enema or prescription laxative and fluids to help hydrate him and aid in stool passing. They can also check and treat for intestinal parasites.
