I am so sorry your picture did not show up immediately when opening your question. It looks like your pet may have an anal gland abscess here. If that is the case and it still isn't resolved, you will need to have the area lanced and drained, if not done so already. Then she will need to be placed back on antibiotics and anti-inflammatory medication. If this is not the problem at hand, then please repost with more information about what specifically is going on with your pet, what treatment was tried and the fact that she is not getting better.
