My cat has bloody urine for a month. Is it cancer or serious?

Updated On September 23rd, 2025

My cat is having hematuria and has been for over a month now. He has been on two different anabiotics prescribed by the vet however we have not done a urinalysis yet. I have a sample in my refrigerator to take to the vet tomorrow, but for the time being I am stressing that it could be something more serious than a UTI. What would other signs and symptoms be if my kitty had cancer or something worse going on?

Hello and thank you for reaching out to Petco Pet Education Center, formerly Petcoach. If your cat is having bloody urine, this could be due to a UTI that is not being treated with the correct antibiotics. It is always recommended to have a urine culture and sensitivity performed to determine what bacteria are present and to what antibiotics they are susceptible. Other causes of hematuria include bladder stones, which can often be diagnosed on abdominal x-rays but can require abdominal ultrasound to definitively diagnose. Stones can be the reason that a UTI does not resolve even with antibiotic treatment, so they need to be ruled out. An abdominal ultrasound would also help a vet see if there is any evidence of cancer within the bladder. Bloody urine can also be seen with feline idiopathic cystitis, or FIC, which is a sterile inflammation of the bladder and lower urinary tract, typically caused by stress. There may be no other signs besides hematuria and straining to urinate with either stones and/or bladder cancer, so an abdominal ultrasound will help the most. I hope this is helpful!
