Why is my dog sick and unresponsive after Vetoryl for Cushing's?

Pet's info: Dog | Labrador Retriever | Female | spayed | 13 years and 9 months old | 62 lbs

Reply: Neneh has been drinking excessive, peeing frequently, up betwern 9p & 2a panting heavily, hungry. &/or wanting ice. She had the LDDS test: Baseline 7+ 1st 36+ 2nd 38+ She also had abdominal ultrasound, both adrenals enlarged, liver mass left lobe, renal issues. Very slight heart murmur, good blood pressure, K+ abnormal, 63 lbs pre-vetoryl. Put on 60mg after breakfast. At 10th day after dose she would hardly eat. Next day threw up & unresponsive & rushed to ER.

Poor Neneh! I'm sorry to hear that she's having this problem. Based on your description, I would say she definitely needs an ACTH stim test to check her cortisol levels - occasionally, cortisol can drop to dangerously low levels after starting Vetoryl or other medications for Cushing's disease, even at normal medication doses. It's also possible that her current illness is something entirely unrelated, so general lab work and a good physical exam are also important to get a full picture of what's going on. If she does have low cortisol levels, this can usually be corrected with IV fluids and a short-term course of steroids. Her Vetoryl can then be restarted later on at a lower dose.
