Poor Jack! These are very concerning signs, especially in a puppy this age - I would be very concerned that he may have a serious illness such as parvo, or an intestinal obstruction due to something he may have swallowed. I would recommend taking him to the vet immediately for an exam, as this may be a life-threatening problem - the vet can do blood work or other testing if needed to help diagnose the problem and get him started on appropriate treatment.
