My kitty has been scratching himself lately non stop, he has caused open sores on his skin and hair loss in several places, I have switched his food but cant get him out of the house to see a vet, not sure what I should do. He just sleeps all day and seems irritated when hes awake.

An itchy cat is almost always fleas, even if you don't see them. I suggest you treat him for fleas if you aren't already doing so. If you are using a monthly flea prevention then maybe the product you are using is not working for him. I suggest you use a quality product like Advantage II. https://www.petco.com/shop/en/petcostore/brand/advantage Don't forget to treat the environment. If no improvement after a week or so of treatment then he will need to see a vet. Hope this helps.
