I am sorry your gecko is not feeling well. It is very hard to tell what is going on based on the picture. It is possible it has a skin infection, burn, problem shedding, prolapsed rectum or fungal disease. I would recommend takin him to a vet for a closer exam.

    It looks like he has a sperm plug stuck over the vent. He could also have an infected hemipene. I would recommend soaking him in warm water for about 10 minutes and then gently try to remove the plug. If you cannot, or that does not rectify the issue you need to get him to a vet. He may need more aggressive treatment especially if an infection is present. I hope this helps. Best wishes!
