My dog has heartworms, vomiting, lethargic. Is it an emergency?

Pet's info: Dog | Dachshund | Female | spayed | 10 lbs

My grandmother has a dog. Her name is Pepper and she is a weenie dog. She is around 9 years old and has been tested positive for heart worms. Today, my grandmother called me explaining that pepper was fine this morning and then all of a sudden she had no control over her bowels and vomited. Pepper also didnt want to move. She refuses to take her to the vet because she believes there is nothing they can do. What can we do?

Pepper has to see a vet immediately. The only way to know for certain if there is something that can be done for her is to have her seen by a vet. It's also not fair to leave her in this condition, she is likely very distressed. Please take Pepper into a vet immediately, this is a medical emergency.
