My puppy is pooping blood, vomiting, and has worms. What to do?

Pet's info: Dog | Great Dane | Male | unneutered | 1 month and 9 days old | 10 lbs

We have a litter we are caring for now, one pup has been pooping only blood since last night. He was also throwing up. I took the litter in 2 weeks ago because 2 were vomitting/diarhea and gave us medicine for vommiting (cerenia) and diarhea (metronidazole). I called today and they said to just administer the same medication. They were negative for parvo and giardia. We just started to notice what looks like worms in his poop. He's looks like on the verge of death, just want a 2nd opinion.

Unfortunately if you feel your puppy looks like it is on the verge of death the best recommendation I could make is to report to the emergency right away. Severe gastrointestinal parasites can result in profound dehydration and bleeding in some instances. Given your dog is a large breed the worm burden would have to be extremely high for this to be the case but it's still entirely possible. It certainly at the very least sounds like your dog needs some additional diagnostic tests like x-rays, more specific blood work, and more in depth fecal testing. I would also strongly advise that you separate out any healthy appearing puppies from sick ones in order to prevent any transmission of communicable disease.
