My cat is pooping blood and worm-like things. Should I worry?

Pet's info: Cat | Oriental Shorthair | Male | neutered | 17 years and 2 months old | 10 lbs

My cat is a 16yo male oriental and he has been wormed. Occasionally, he will defecate a few times and then these worm like structures will come out at the end. He then keeps going back and forth to the toilet and sometimes small droplets of blood will come out. I am not sure if it is from his anus or penis. I am starting to get worried.

That is very concerning, and since the blood is bright red I'd be worried that there is mucosal linings (what you think are worms) that are being shed from the lower GI tract leading to the bleeding due to inflammation or an illness. I would seek out care from your vet (or an ER clinic if he seems in distress currently) right away and bring the sample with you.
