My stray cat returned injured and thin. What to do about kittens?

Pet's info: Cat | Domestic Shorthair | Female | unspayed | 6 years and 9 months old

We have a stray cat whom we are feeding since her childhood. Few weeks ago she hsd a little bump in her belly so we suspect she is pregnant. She went missing 2 weeks ago and suddenly showed up this evening. She was all in dirt,having a small wound on her nose and is extremely feeble and thin. She have eaten very little,and while cleaning her up i saw little blood near her private part(Now its dry and clean) and a little while ago she was meowing very loudly this at midnight.What should i do?

She may have had her kittens, so you should let her go out if she want to, Leave food out for her and she will be back. If you put a box out next to the door (box closed with an entrance, and a towel inside) she may bring the kittens home. Get some kitten food for her to build her condition up. You may need to take her to a vet if she does not get better. Check her teats whether she is lactating.
