Why is my dog still itchy with a worsening lick wound on her paw?

My dog is always itchy so we treated her for fungal infection and all her wounds disappeared. But she's still very itchy. She doesn't have a single tick. But there is a wound kind of thing on her hand which doesn't go away but with time getting worse. She keeps licking it all the time. Please recommend what should we do about it. And what is it exactly. Pet's info: Dog | German Shepherd | Female | unspayed | 3 years and 4 months old | 61.7 lbs
