Why is my outdoor feral cat which I have fed consistently for months losing weight? He eats a little and moved around and still grooms. Should I be worried?

The simplest reason would be intestinal parasites. They can be treated relatively easily as well. Other reasons would require a physical exam. If he will allow you to place him in a carrier, have him examined by your veterinarian.
