About 8 months ago, I brought my 2 year old male neutered cat to the ER vet because he was frequently trying to urinate (only on my rug and not in his litter box) but the only thing that would come out was drops of blood and he was crying in pain. After tons of tests and bloodwork they treated it as a urinary tract infection and now I only feed him Urinary tract health food. Now, all of a sudden he has only been peeing on my rug (but pooping in litter box) regardless of how clean I keep his box.

I would recommend rechecking with your veterinarian. Cats can have recurrent episodes of feline cystitis (inflammatory bladder disease) or less commonly infections. If the inflammation or infection isn’t treated it can lead to a urethral obstruction. It’s possible that he needs medication to help him through this current episode.
