What's causing my dog's red testicles when vets can't diagnose?

Pet's info: Dog | German Shepherd | Male | unneutered | 3 years and 4 months old

My vet is not a ke to diagnose what this is going on all blood reports came normal also consult for second opinion and still this is like this earlier it used to be black now its red his lips also turn red from black cephalexin is not working on him neither itraconazole is working on him so what this disease is all about kindly check attach photos and pls give me idea

This could be a bacterial infection that is resistant to cephalexin (or just needs longer on cephalexin if it improved and then worsened). If the redness and moisture is only on his testicles, I would also be concerned that something is causing him pain in there (cancer, twisted testicle) and causing him to lick excessively and damage the skin. Even a simple infection would be difficult to get to resolve if he's licking at it. There's nothing obvious jumping out at me from the picture. Since you've already had two opinions and tried a few different empirical treatments, it may be time to biopsy the skin or see a veterinary dermatologist. Good luck!
