My dog's penis won't go back in after mating. Is it infected?

Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Male | unneutered | 3 years and 2 months old | 28 lbs

My dog is not neutered (vasectomy done 2 year ago). 7 days ago after a long mating session, his penis didn't go back to its skin. The vet suggested a topical betamethasone, which didn't help. The penis now has discharge and infection has damaged the tissue. It has turned black in the base (couldn't get it in the pic). The vet thinks there is no hope since the tissue is damaged even tho he is on antibiotics and the infection is under control. I wanted a second opinion as we are out of the country

This does indeed look like your dog's penis is seriously hurt. There is a possibility there is a fracture of the penis bone, beginning necrosis. It may be a good idea to do an x-ray of the penis, to determine if there is a fracture as well. If there is extensive necrosis, then his penis may have to be amputated and since you say that he was vasectomised rather than castrated, ie still has his testes, it would be a good idea to have them removeed as well. The testicles will keep making testosterone, which will make him seek out females and want to mate, Even if the penis heals, I do suggest you have him castrated to avoid further matings and possible injuries.
