My dog's paw pads are swollen with white spots. Is it an infection?

Pet's info: Dog | Mixed Breed Small (up to 22lb) | Male | 18 lbs

My dog has, what appear to be, very swollen areas on/under his foot pads with small white-head like spots. They seem to come and go but have become progressively worse. His primary vet has seen them but has dismissed them as common/not painful. I fear he may have a deep infection as the afdected areas seem very swollen/hard and don’t seem to be getting better. He is a 9 year old terrier mix with chronic kidney disease so certain

I am sorry to hear about Scrappy. These are unusual lesions and not like anything I have seen. If your vet has not done any testing on them that would be the next step, either a needle aspirate or a biopsy to ensure this isn't something that may require surgical removal or medical intervention, particularly since they are worsening and these are taking up a substantial portion of the paw.
