Puppy eating grass, vomiting, diarrhea, lethargic. Vet needed?

Panda has been eating lots of grass and vomiting and has slightly loose bowels he is a bit lethargic and isn't keen on eating does he need to go to vet or can I great him at home. Pet's info: Dog | Mixed Breed | Male | neutered | 4 months and 20 days old

In a young pup, a full check up would be advisable given both vomiting and diarrhea. If he is bright and alert you could try a bland, intestinal prescription diet and a probiotic for no more than 48hrs without improvement
