My dog's diarrhea isn't improving after vet treatment, what's wrong?

Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Female | spayed | 13 years and 1 month old | 45 lbs

My dog had diarrhea and nausea a few weeks ago. I took her to the vet who gave her some medication and a strict diet to follow for a few days. Her symptoms cleared up almost immediately. A few days ago her diarrhea returned but she has no other symptoms and is acting normally and eating normally. The day after it returned the vet gave her a the prescription food and a probiotic like she had before. This has been going on for 4 to 5 days, and has not cleared up or changed. What could be wrong?

These signs could be compatible with an inflammation infection of the gastrointestinal system and surrounding organs (like pancreas and liver) a bit more severe than your vet initially thought. Another clinical examination at your local veterinarian is immediately essential to confirm these clinical suspicions and treat appropriately. Further investigations (like abdominal imaging as ultrasound or radiographs) and possibly antibiotics may be needed this time.
