My dog's diarrhea isn't improving after vet treatment, what's wrong?

My dog had diarrhea and nausea a few weeks ago. I took her to the vet who gave her some medication and a strict diet to follow for a few days. Her symptoms cleared up almost immediately. A few days ago her diarrhea returned but she has no other symptoms and is acting normally and eating normally. The day after it returned the vet gave her a the prescription food and a probiotic like she had before. This has been going on for 4 to 5 days, and has not cleared up or changed. What could be wrong? Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Female | spayed | 13 years and 1 month old | 45 lbs
