My puppy has yellow, soft stool with mucus. Is it diet or vaccine?

Hello. My puppy Patch seems to be all active and very hyper. Today morning his stool was soft with outer mucous coat and yellow. Until last night it was formed stool and brownish in colour. His diet is chicken rice and milk. I got him home a week back from a family where he ate the same diet so I didn't change his diet till now. He got his first vaccine two days back. Is it because of the vaccine shot or because of the diet? He also eats plaster off the wall? How to prevent that?

Diarrhea is a signs referring to a intestinal problem. It could be due to dietary indiscretion, parasitic infestation, bacterial diseases or viral diseases especially because Patches is not fully vaccinated yet. 
You were doing good by providing bland food (boiled breast chicken and rice) but I would't recommend milk (they cannot digest it and could be the cause of the diarrhea). Given his young age, if he continues to have diarrhea, could easily get dehydrated. 
If you can spot blood in the faces or if he get lethargic and not eating, or if he start vomiting, I would suggest to have him checked by your veterinarian as soon as possible. 
 Even eating plaster off the wall might cause intestinal upset, try to avoid it by distracting him with toys or treats (or toy where you can hide treats to keep him busy!). When is back to normal self, I suggest to use a good quality commercial diet as chicken and rice isn't a complete and balanced diet for a puppy.
