My dog's head is tilted and he keeps shaking it. Should I worry?

Pet's info: Dog | Mixed Breed Large (61lb +) | Male | neutered | 6 years and 5 months old | 95 lbs

My dog is 88-year-old rescue dog and I'm wondering if he's OK. His head is slightly shifted and he keeps shaking his head. I'm wondering if this is anything major or nothing to worry about .

It's most likely an ear infection, if the head is tilted it means the infection is already quite severe and deep. You need to bring him to the vet ASAP in order to get treated, he will need antibiotics and ear drops to solve this problem.
