I have a puppy that seems to be about 4 or 5 months old not sure and he needs his shots how do I determine his age and what vaccine he needs

The easiest way to age a puppy is looking at their teeth. The top central incisions generally get replaced by adult teeth around 14-16 weeks. Typically puppies will need a minimum of 2 sets of vaccines depending on the age they were started as you want the last set to fall after 16 weeks of age. They usually get a rabies vaccine by 12-14 weeks and your vet has to administered that.
