My male cat can't pee, has a hard belly, and is in pain. What's wrong?

Pet's info: Cat | Siamese | Male | unneutered | 2 years and 4 months old | 10 lbs

He is not eating or drinking, is unable to go to the bathroom at all, and has a hard ball in his lower abdomen. And of course he seems to be in a lot of pain. He is a 2 year old Siamese, and had tape worms for a long time before I was able to find anything that I could get him to take for it. He was very active, and suddenly became lethargic. He was throwing up to begin with. Is this intussusception?

Oh dear! From your description, I would be very concerned that Teddy has a urinary obstruction - this is a common problem in male cats, and can cause all of the symptoms you described. The overly full bladder often feels like a hard ball in the abdomen. This is a medical emergency, as the bladder can rupture and cause death in a matter of hours if the obstruction is not relieved. I would recommend taking Teddy to a veterinarian immediately for treatment - he/she can sedate him to pass a urinary catheter and relieve the obstruction, and will likely also need to keep Teddy in the hospital for IV fluids and supportive care for the next day or two.
