Why is my Shih Tzu constipated with bloody stool and sore bum?

My dog was at the vet on Tuesday she had a slight temperature but is on antibiotics she's a little disinterested in her food but will eat out of the hand but she cannot poop and when she does it's little and has blood in it and very dark her bum looks very sore and red and is itchy she is a shitzu. Pet's info: Dog | Shih Tzu | Female | spayed | 18 lbs

This is very concerning, the dark colour indicates she may have a bleed in the gastrointestinal tract. Urine blood tests for medical issues such as kidney and liver disease may be required as well as an abdominal x-ray for any signs of obstruction or constipation. Intravenous fluids may be required for dehydration also. Please schedule a recheck as soon as possible
