My dog's eye ulcer looks worse on medication. Is it a perforation?

Pet's info: Dog | Shih Tzu | Male | unneutered | 14 lbs

Could my dog still have gotten a corneal perforation even though he was taking medication for the ulcer? And the ulcer looked to be superficial this morning.

Yes, it's definitely possible, medication for corneal ulcer do not always solve the problem unfortunately, you need to take him back to the vet urgently, he might need a surgical procedure in order to save the eye.
