No they cannot be pregnant and in heat at the same time. I really hope you don't have a pregnant 4 month old cat. Most cats do no go into heat until they re 6 months or older. Cats should not be bred until they are at least 18 months old. If you are not planning on breeding her I suggest you have her spayed as soon as possible.
