Poor pup. This looks highly suspicious for either demodexx mites or scabies mites. The next step here is to get your vet to test Buddy's skin. A simple skin test should be able to tell which mite is causing this and then get him on the proper treatment to rid the mite. He also has a severe secondary bacterial infection that needs to be treated with a long course of antibiotics prescribed by your vet. Most pets with a mite infection recover well after starting treatment, but in severe cases like this it can take 1-3 months to fully rid the mites. I do hope that Buddy's skin improves soon!
