My dog licked his belly red and inflamed. Does he need antibiotics?

He started licking continuously at his abdomen and he does early in the morning when everyone is asleep so when I see it today I found this redness and rough inflamed skin with skin like hives and dry skin so today I apply topical creams and miconazole spray as well so by looking at attach pictures what do you say does he also needs and antibiotics course ? Pet's info: Dog | German Shepherd | Male | unneutered | 4 years and 2 months old

I suspect Bruno needs antibiotics and anti-inflammatories. That lesions look very red. He shoudl also wear an elizabethan collar at all times to prevent licking which could make it worse. I hope this helps.
