Dog has protein in urine and high blood pressure. What meds help?

Updated On September 23rd, 2025

Pet's info: Dog | Scottish Terrier | Female | spayed | 12 years and 3 months old | 36 lbs

My white Scotty Terrier, 35 pds. Has a urine protein creatinine of 2.4. What medication would be helpful to decrease this? Roxie dog also has borderline high BP. Lisinopril or Rafampin? I am an RN and I am familiar with these medications. I will discuss this with my Vet (who always chooses the higher cost med provided from her office). I would like an opinion for another Vet. Thank you!

If a UTI has been ruled out by urine culture, then protein losing nephropathy can be suspected as the cause, and an ACEI such as enalapril or benazepril can be used. I believe there is a newer medication on the US market for this purpose in dogs, but I can't remember it now! I just learned about it at a lecture a few months ago. I don't typically treat borderline high BP, but if the PLN is real then the ACEI will certain help lower the BP to decrease protein loss. Beta blockers would be the next step, but I haven't needed to use them personally.
