My dog is vomiting, X-ray shows issues. Is surgery really needed?

Pet's info: Dog | English Setter | Male | neutered | 1 year and 7 months old | 45.8 lbs

I took my dog to the vet today, he had thrown up once on Monday, 3 times yesterday, and twice this afternoon. This afternoon he threw up a blade of grass, but all times he threw up beforehand it was just yellow liquid. The attached photo is an X-ray of his abdomen and the vet is recommending surgery - just wanted to get a 2nd opinion! Thanks!

I can see a few areas in the x-ray that would concern me although I don’t think there’s an obvious foreign body or obstruction. It might be more obvious in person - it’s difficult to look at x-rays on a phone. Any time a dog is continuing to vomit despite appropriate treatment with anti nausea medication I get concerned, and it sounds like exploratory surgery might be the next step.
