My dog's paw keeps getting injured. What's causing this chronic problem?

Pet's info: Dog | Australian Shepherd - miniature | Male | neutered | 3 years and 8 months old | 17.63 lbs

My dog often injured his paw on the same place. We usually apply Betadine and a cream, and bandage it. It goes away after a while but tends to reappear. However, today the wound looks worse than it usually is. What could be the reason behind this? And what can I do to prevent it?

This is not a recurring injury. This looks like foxtail/grassawn lesion. There is likely a piece of grass up there that is causing a chronic infection.
