My dog's teeth look stained. Is it a cavity? What should I do?

This looks like dental staining. She does have gingivitis and dental plaque so I recommend a dental cleaning. When the teeth are scaled and polished the stained area may disappear. Also when they are working on her teeth they can check this area more closely to see if any additional treatment would be recommended.
