Why is my senior cat shaking his head, twitching, and scratching ears?

Pet's info: Cat | Mixed Breed | Male | neutered | 9 lbs

Hello. I have a 13 year old cat that is very healthy. I have taken him to my vet before about this issue but she said it was nothing. Probably fleas. He shakes his head and sometimes lowers his ears down, squints his head and like twitches his head. He doesn’t have fleas especially right now but I also don’t see signs of mites like my vet said. But he will scratch all day at his ears and behind the ears. Should I go get a second opinion?

I would definitely have his ears swabbed and have them look under the microscope for signs of an infection. Otherwise, this could be a sign of a neurological issue and he may need to see a specialist.
