Why is my dog losing so much hair and scratching after meds?

Hi. I have. 3 year old field spaniel who is loosing huge amounts of hair. He was treated for an ear infection in Oct with steroids and antibiotics and it seems to have started then and is not slowing down. mainly on inner thighs and tummy and also around neck, chest is thinner. He is scratching, otherwise he has no other symptoms. Photos below are this week and 2 months ago. Pet's info: Dog | Field Spaniel | Male | unneutered | 3 years and 4 months old | 40 lbs
