Why is my dog's penis swollen and sticking out while he's resting?

My dog was resting on bed but started licking and I noticed his penis was swollen and stuck out some. I immediately ran water over it and it shrunk and I got it back in. He wasn’t simulated prior to this event. What could cause it to happen while resting? I did trim the hair around that area after it happened. He had a good vet check up last week. Thanks. Pet's info: Dog | Shih Tzu | Male | neutered | 9 years and 1 month old | 12 lbs

That can just happen if he is licking the area. I would not be concerned unless he seems in distress, or he is unable to retract the penis on his own/it is completely swollen or causing pain. If that is the case, seeking veterinary care in person right away is best.
