Is it time to humanely euthanize my very sick, suffering senior dog?

Question

my dog has diabetes, bushings, blind, deaf and the beginnings of neuropathy. She is 14 years old and is currently taking 13 units of insulin twice a day. She doesn't like to walk and I have to carry her up and down stairs. She has constant ear infection and the odor is terrible. She is losing her coat and sleeps almost 19 hours a day. I want to put her down. It is time. She has no quality of life. Pet's info: Dog | Bichon Frise | Female | spayed | 4 years old | 24 lbs
