My female dog has pyometra. Is surgery needed or can medicine work?

It looks like from the ultrasound report that Lucy has a pyometra. This is a severe infection of the uterus that is fatal without treatment. I agree with your vet's recommendation to have her uterus removed surgically (spay her). Treating a pyometra with antibiotics is typically not effective, and the recommended treatment with the best results is spaying the dog. I would follow your vet's advice and have her spayed asap. Leaving her with a pyometra will make her sicker and sicker, time is of the essence in treating this infection. Best of luck.
