my cat initially squats to urinate in his litter box, then he stands up straighter and the urine sprays backwards while his tail is wildly vibrating. Needless to say, the urine goes everywhere. The litter box is clean.

This sounds more like territorial spraying (the tail action is indicative), it is more common in mutlicat or busy households and it can help to provide feliway diffusers and a calming supplement such as zylkene. Try washing down affected areas with biological laundry detergent and not bleach as this can encourage re-marking of areas
