The problem may lie in the way you keep your dog, There may be bacteria, parasites or allergens in that straw. Remove the straw, vacuum and disinfect the area and give your dog a nice blanket to lie on. Take him out for exercise and let him run. Feed him a good quality dog food. Regularly wipe his skinfolds on the head body and around the tail with with wipes to keep them clean. This, in combination with the medications given may start making a difference. If not, you may need to get a referral to a veterinary dermatologist.
