My cat shakes his head, scratches his ear, and meows. What's wrong?

I have a male black and white cat he is 10 years old. I have noticed maybe the last two weeks on and off and last month once in a while. He will shake his head and scratches his ear and meow out loud are growl. What is making my cat doing this? I am very concerned about my boy. Thank you. Pet's info: Cat | Domestic Shorthair | Male | neutered | 15 lbs
