My male dog's penis is stuck out and his scrotum has a painful rash.

My dog is 11 and has a history of UTI he’s currently taking a preventative and curing medication for dogs with UTI. His penis has been like this for a few weeks and we originally thought that it was heat ( never spayed) but I did some research and learned that it was more than that. Today I noticed this rash/ blemish that was causing him some pain on his scrodum. I wiped the aware with warm water and mild soap and put a cone on him and that seemed to ease the pain. Should I be worried?

That needs to be seen by a vet immediately. Male dogs do not go into heat, only female dogs do this. It looks like he is suffering from paraphimosis, which is an inability to retract the penis back into the sheath. This can quickly turn into an emergency situation, as constricting of the blood flow will lead to a larger engorgement of the penis, necrosis (dead tissue), and potential damage to the urethra. Please take him into a vet now.
