Why does my German Shepherd have scabs under ear and a head bump?

Hello my name is Daniel and my parents refuse to take my dog to the vet so I wanted to ask if you could help figure out what’s wrong with him? He has severe scabs under his ear and he just grew a bump on his head. The scabs have been there for about a year now and we have tried different medications and they don’t work. Also, forgot to mention the dog is a German shepherd and is about 11 years old. If we can’t figure out what’s wrong soon we might have to put him down.

This presentation is due to a skin infection, possibly caused by bacteria. A veterinary consultation is needed in these cases as antibiotics will be quickly resolving this problem. Disinfection may be started at home with very diluted hibiscrub or betadine or salted water baths. Buster collar may be useful as well to avoid any licking/scratching (if any).
