My dog's ear infection won't go away after drops. What's wrong?

Vet in Mexico told she had a ear infections gave drops for 7 days but she still scratching and has a odor. Pet's info: Dog | Shih Tzu | Female | spayed | 11 years and 1 month old | 12 lbs
