My senior dog needs costly vet care. How can I afford treatment?

my girlfriend has a Maltese and Pekingese mix named Buzz that is 13years old. About three years ago he had a hernia in his neck and we spent about 4500 to have surgery. Surgery was successful and we haven't really had any problems since but the vet warned us that he has a few places which will likely cause a problem in the future. I think he is having issues again. If I can't find a cheaper vet and cannot pay another 4500 to fix it what other alternatives are there for our family? Pet's info: Dog | Mixed Breed Small (up to 22lb) | Male | neutered | 13 years and 2 months old | 20 lbs
